If you’ve received a referral for a client (either directly or from a contact such as a support coordinator or GP), please follow these steps:
1. Send the Service Enquiry Form
Share the following link with the client or the person making the referral:

This form is essential as it sets up the client profile in our system and allows for scheduling the initial consultation.
2. Notify AHA Assist (Admin)
Log a ticket with AHA Assist to let admin know to expect a referral for you and we'll keep a look out for it. Once the form is submitted, our team will review the details and reach out to arrange the first appointment.
Please note: One form must be completed per participant.
